If safety is important, then there should be common standards. However the medical requirements (eg the need for an ecg) should be evidence based.
I remember a few years ago a racer and heart surgeon of my acquaintance challenged the MSA (as it was then) and the FIA on the need for a stress ecg for an international C licence as he reckoned it was more likely to initiate a heart attack than prove that you were unlikely to have one.
