when was the last time a team failed to field two cars in quali, in a race, or for both?

I remember it happened to Toyota when Glock crashed in Q2. They had to scratch the car so the race started with 19 cars.

I dont think that was a car issue but more a driver one, Glock being declared unfit, and a substitute driver unable to take the seat because they had not taken part in qualifying.

There was an interesting similar case in Paul Ricard 1984 when Andrea de Cesaris had all sorts of problems and qualified 27th, when only the top 26 could qualify. So Ligier withdrew Francois Hesnault, who had qualified 14th, so that De Cesaris could start from 26th. De Cesaris finished 10th.

Kajima entered qualified and started two Japanese drivers (Noritake Takahara and Kazuyochi Hoshino). Hoshino qualified 11th and Takahari 19th of 23 starters. (A 3rd Japanese driver, Takahashi, also started 23rd for Tyrrell in a third privately entered Tyrrell. He finished 9th)
The two Kajima drivers had different fortunes....Takahara tangled with Andretti and Binder, out of the race on lap 1 with Hoshino finishing 11th.

They ran as #50 for Takahashi with the two Kojimas Takahara #51 and Hoshino #52.

Yes, in 2006 Takuma Sato had Yuji Ide as teammate the first four races and Sakon Yamamoto for the final seven races.
